DiD is looking for experienced NDIS Support Coordinators and Psychosocial Recovery Coaches to join our team on a contract based in Adelaide SA.

Role Description

This is a contractor role for a Support Coordinator/Psychosocial Recovery Coach. The Support Coordinator will assist NDIS participants in planning, connecting, and coordinating their supports and services. The Support Coordinator will also help with developing individualized plans and monitoring their progress to ensure participants achieve their goals.

  • Collaborating with participants to develop and implement high-quality NDIS plans that take a holistic approach to addressing their needs
  • Coordinating and facilitating access to a wide range of support services, including allied health, community programs, and housing assistance
  • Walking alongside participants to ensure their rights are protected and their needs are met
  • Providing psychosocial support and coaching to assist participants in their recovery and personal development

Qualifications

  • Strong Interpersonal Skills and Communication skills
  • Experience in Support Coordination, Customer Service or related fields with transferable skills
  • Strong knowledge of the NDIS
  • Ability to work independently and remotely
  • Tertiary qualifications in Social Work, Psychology, Human Services, or related field is desirable
  • Lived experience is welcomed and encouraged
  • Compassion, empathy, and a genuine commitment to improving the lives of people with disabilities and mental health support needs
  • Have excellent organisation, communication and time managements skills, follow up and get the job done!
